    Employees can purchase of up to an additional $40,000 insurance at low group rates.
    Supplemental Life Insurance Same as Health Insurance You Same as Health Insurance Employee can purchase from several levels of life insurance amounts. Premiums are on an after tax basis. Coverage also available for spouse and dependents.
    Accidental Death & Dismemberment Insurance Same as Health Insurance You Same as Health Insurance Employees can purchase for themselves and family from five levels of life insurance ranging from $50,000 to $250,000. Premiums are on a pre-tax basis.
    Child Care Reimbursement Same as Health Insurance You Same as Health Insurance Allows employees to pay for Child Care expenses on a pre-tax basis.
    Health Care Reimbursement Same as Health Insurance You Same as Health Insurance Allows employees to set aside pre-tax dollars to pay for Health Care expenses not covered by health insurance.
    Funeral Pay Account Same as PTO SMH-100% Upon employment Up to three days may be allowed at the time of a funeral for a member of immediate family.
    Tax Deferred Annuity All employees You Upon employment You may purchase a Tax Deferred Annuity through payroll deduction on a pre-tax basis.
    Pension Plan Employees who work 1,000 or more hours per year SMH 100% Age 65. Employees may begin receiving pension benefits as early as age 55. Retirement income to supplement your Social Security Benefits.
    Social Security All employees SMH 50% You 50% Upon employment Financial assistance at retirement or if totally disabled.
    Travel Insurance All employees SMH 100% Upon employment Life insurance coverage ($100,000) while traveling on SMH business.
    Worker's Compensation All employees SMH 100% Upon employment Medical care and compensation for on-the-job injuries.
    Employee Assistance Program All employees SMH Upon employment Basic counseling services to assist employees with personal problems, e.g., financial, marital, chemical dependency, etc.
    Personal Time Off (PTO) All regular, full-time & part-time employees (Part-time employees receive PTO on a pro-rated basis) SMH 100% Accrual begins the first day of employment. Can be used after 90 days of employment. Personal leave days for holiday, vacation, personal and short-term sick leave.

    Leave of Absence All regular full-time and part-time employees SMH After 90 days of employment Up to one year off with retention of accumulated benefits.
    Military Leave (Reserve Annual Training) Full-time employees SMH Upon employment Base pay minus military duty pay for hours absent to attend annual training.
    Jury Duty All regular full-time and part-time employees SMH Upon employment Base pay for hours absent.
