How to Give to St. Mary's Hospital

The Friends of St. Mary's Hospital foundation is a division of the Hospital Sisters of St. Francis Foundation, Inc. This foundation is a nonprofit organization chartered in the State of Illinois and is exempt from income tax under Section 501 (c)(3) of the United States Internal Revenue Code. Therefore, contributions to the Friends of St. Mary's Hospital qualify as charitable contributions to the fullest extent of current laws.

In addition to outright cash gifts, other giving options available for donors include the following:

• Bequest in One's Will or Other Estate Plan Documents
• Real Estate
• Life Insurance
• Marketable Securities
• Charitable Lead Trust
• Charitable Remainder Trust
• Individual Retirement Account

Donations to the hospital in the form of a check can be made out to "The Friends of St. Mary's Hospital" and mailed to the following address:

Donations can be designated in one of two ways:

• Unrestricted Gifts
  Gifts may be made to the Friends of St. Mary's Hospital with no restrictions as to their use. Unrestricted gifts will be used where the hospital's needs are the greatest as determined by the hospital Administrator and reviewed by the Friends of St. Mary's Hospital Board of Directors.



• Restricted Gifts
  Although unrestricted gifts provide the greatest flexibility to the hospital in carrying out the health care ministry, a preference may be expressed for the general type of programming to be supported by adding, "To the extent possible and practicable, this gift shall be used for the area of ____________."
