Conveniently located in the Forsyth Commons shopping center, St. Mary’s Family Medicine Forsyth focuses on health care for your family. Three board certified nurse practitioners are available:

  • Theresa Henderson, FNP specializes in diabetes, disease prevention, health promotion and women's health.
  • Jennifer Ross Barnett, FNP specializes in adolescent care, early childhood care, preventative care and women's health.
  • Carolyn Wagner, FNP specializes in management of acute and chronic health needs, preventative care, pediatric care and women's health.

    They work together with Marlon Muneses, MD whose main office is located at 1770 E. Lake Shore Drive, Suite 207, Decatur, Illinois, 62521, (217) 429-9700.
