St. John's Hospital Home Health Services provides home health and hospice care in the home as prescribed by a physician. St. John's Home Health Services is conveniently located on the campus of St. Mary's Hospital.
Home Health Services can help promote, maintain or restore health, or provide pain and symptom control. We provide services to acutely or chronically ill patients and to patients recently discharged from the hospital who are in need of nursing care and/or restorative therapy. Offices are located in the Springfield and Decatur areas. There is no restriction by diagnosis or age group.
Home Health Skilled Services are covered under Medicare and Medicaid, subject to guidelines. Most insurances cover Home Health and each insurance benefit is clarified prior to service. Individuals may pay privately for Home Health Services

Skilled Intermittent Program
St. John's Home Health Services is an attractive alternative for the participant who requires skilled care on an intermittent basis. Medicare and Medicare and most third party payors cover home health services (subject to guidelines). Individuals may pay privately for Home Health Services.
Home health services consist of:
- Medical supervision provided by the patient's attending Physician in consultation with the home care team.
- Skilled nursing provided according to a physician's order. Specialized care is available for patient's receiving home intravenous therapy, chemotherapy, total parenteral nutrition, post-surgical care, cardiac rehabilitation, pulmonary rehabilitation, wound care, and pediatric care. In addition, health education is provided for patients and their families.
- Home Health Aides, certified by the state of Illinois, provide personal care
(such as bathing and grooming), assist with therapy exercises and assist home
health nurses in providing patient care.
- Physical Therapy assists the patient with prescribed exercises and activities. The therapist can teach the participant, family or health care aide to help with appropriate treatments.
- Social Work Service provides counseling to the patient and referral to community agencies and resources.
- Speech Therapy provides a program of treatment that includes therapy as well as follow-up on the participant's progress.
- Occupational Therapy helps the patient in overcoming or living with disabilities. The therapist can teach the patient and family ways to adapt activities of daily living such as bathing, cooking, dressing, etc.
Certification
St. John's Home Health Services is licensed by the Illinois Department of Public Health as a home health agency. Home Health Services is also certified for participation in Medicare and Medicaid programs and accredited by the Joint Commission on Accreditation of Healthcare Organizations.
Hours of Service
Home Health Services are available 7 days a week, with a Home Health nurse on call 24 hours a day to assist the patients as needed.
Referrals
Referrals to St. John's Home Health Services are accepted from physicians, hospitals, nursing homes, community agencies, and members of the community. Clarification of the services needed by the participant is always obtained from the attending physician prior to provision of care. Cardiac Rehabilitation Program
St. John's Home Health Services has developed a cardiopulmonary rehabilitation program to help patients and families with heart or lung disease recover and return to optimal levels of functioning.
A nurse, with training and experience in cardiopulmonary rehabilitation, works with the Physician, patient and family to develop an exercise program suited to the individual patient.
Home Cardiopulmonary rehab services include:
- Assessment
- Personal exercise training using safe exercise protocols, including access to cardiac monitoring and monitoring of oxygen saturation levels
- Education about medication, risk factors for heart disease, lifestyle modifications, nutrition and stress management

Psychiatric Home Health Services
St. John's Hospital Psychiatric Home Health Services is a program for the home-bound patient requiring short-term, intermittent psychiatric/mental health care. It is provided under the direction of a physician as a component of the continuum of care for patients with psychiatric disorders.
- Increased patient compliance with medication, treatment plans, physician visits and other follow-up care
- Earlier discharge from inpatient stay
- Evaluation of home/environmental factors contributing to the patient's illness
- Management of medical needs, as well as psychiatric needs, with nurse acting as liaison between patient and physicians
- Qualified psychiatric nurses are available Monday through Friday (8 a.m. - 4:30 p.m.), with on-call services provided 24 hours a day, 7 days a week
- Coordination of community resources and services
Criteria for admission may vary according to payor source. In general, patients must suffer from a psychiatric disorder that is acute in nature or an acute exacerbation of a chronic illness. The patient must require the specialized knowledge, skills and abilities of a psychiatric nurse, and must be psychiatrically or physically home bound. For the psychiatric patient, this often is exhibited by the patient's refusal to leave the home, or by an illness of such nature that it would be unsafe for the patient to leave home unattended.
The Psychiatric Home Health Program is covered by Medicare, IDPA and some private insurance companies. These agencies are billed directly for our services.
Patients appropriate for Psychiatric Home Health include those of any age with a psychiatric diagnosis including:
- Mood Disorders
- Thought Disorders
- Anxiety Disorders
- Gero-Psychiatric Disorders
Those patients with acute psychiatric needs related to their health status, and those experiencing impaired individual and/or family coping mechanisms related to a health crisis, grief or loss can also be referred.
The psychiatric nurse provides the following services under the direction of the patient's physician:
- Ongoing psychiatric nursing assessment of the patient's mental status, level of functioning and psychosocial stressors
- Evaluation of the patient's response to treatment
- Medication education; monitoring, set-up and administration
- Patient education regarding the illness, developing healthy coping skills and lifestyle changes
- Supportive therapy
- Ongoing assessment and monitoring of medical needs
- Education and support of the caregiver/significant other
- Collection of lab specimens and monitoring of results, including drug levels
- Referrals to community resources and coordination of services

Pediatric Program
Pediatric Services from St. John’s Home Health Services are performed by a highly qualified staff of registered nurses, therapists and social workers.
Pediatric skilled nursing care includes:
- Treatment and education needed for recovery
- Procedures and education needed for maintenance of surgical interventions (such as gastrostomies, tracheostomies, colostomies, etc.)
- Healthcare maintenance focusing on independence for families of children with illness including asthma, diabetes, failure to thrive, malignancies, cardiac anomalies, syndrome disorders, and neurological disorders
- Home Infusion Therapy (TPN, blood components, antibiotics, fluids, chemotherapy, IV medication)
- IV line care – both peripheral and central venous access
A home health nurse is on call 24 hours a day, 7 days a week.

Hospice Homecare Program
St. John’s healing mission of caring for the sick and injured was initiated over 100 years ago by five German Sisters of the Third Order of St. Francis. Today, St. John’s continues to carry out the Sisters’ commitment of serving the people of God. Our hospice program is a continuation of these efforts, fulfilling its role in the mission by providing for the spiritual, physical and emotional needs of terminally ill patients and their families.
The St. John’s Hospice Homecare Program:
- Provides compassionate, specialized care to terminally ill patients and their families
- Offers a team of physicians, registered nurses, medical social workers, supportive and spiritual counselors, personal care assistants, physical, occupational and speech therapists, registered dieticians and volunteers working together to provide individualized, compassionate care
- Offers support for the physical, emotional and spiritual needs of the patient and families
- Relives anxiety by providing information to the patient and family about what to expect as the illness progresses
- Provides medically managed pain and symptoms control.
Hospice Admission Criteria:
- Diagnosis of a terminal illness due to any disease for which curative treatment is no longer sought
- Reside within the service area if homecare is to be provided
- Order of the attending physician
Hospice Services Include:
- Coordination of all hospice services
- An interdisciplinary team specially trained in providing hospice care
- Registered nurses on call 24 hours per day
- Insurance/reimbursement verification
- Counseling, volunteer and bereavement support.

Wound and Ostomy Care
St. John’s Home Health Wound and Ostomy Care offers:
- Specially trained nurses who will assist with your wound care needs
- Education and training provided to you and your caregiver for your wound/ostomy
- Nurses who will provide skilled assessment of wound healing and communicate findings with your physician as needed
- Discussion of supply needs on an individual basis
- Nurses on call 24 hours a day, 7 days a week if questions or needs arise between visits
